Pentagon Review Blames AI and Intelligence Lapses for Minab School Strike

A Pentagon review found that outdated intelligence, reduced civilian-harm staffing and overreliance on AI contributed to a US strike that killed about 120 children at a school in Minab, Iran. The Tomahawk missile was identified as US-made, and CENTCOM later correlated the strike with US forces. The UN says the strike may constitute a war crime.
