Spectral CT improved differentiation of intrapancreatic accessory spleen (IPAS) from hypervascular pancreatic neoplasms on portal venous-phase imaging, a retrospective study found. Among 85 patients, IPAS lesions showed lower attenuation and iodine density than non-IPAS lesions. Relative lesion-to-spleen ratio yielded higher diagnostic performance than lesion density alone, suggesting spectral CT can help evaluate incidental hypervascular pancreatic lesions.
